At Your First Appointment

  • Arrive 15 minutes early for registration
  • Bring all insurance information. We will assist you in understanding your benefits.
  • Bring a list of all Medications and dosages you are currently taking.
  • We require 48-hours' notice for schedule changes on appointments.
